Health History & Release Form

1. 1. Camp's held in CT or MA require Dr. Signature on the health history & release forms.
2. 2. If your camper takes medication, they must bring the signed prescriber's authorization form along with each medication.
3. 3. If your camper can self-administer their medication (Asthma Inhaler/ Epi Pen), you must fill out the self-administer portion of the authorization form.
